The list
- The application form, MHC/203.
- A passport-style colour photo.
- The medical form, TPH/204, completed by your GP.
- Your birth certificate or passport.
- Your full driving licence.
- Evidence of your right to work in the UK.
- An enhanced DBS certificate, or its application reference number.
- Confirmation that you are aware of HMRC's guidance on tax obligations.
- A marriage certificate or deed poll if your names differ between documents.
- Form TPH/205 and certificates of good conduct, if you have lived abroad.
Every given name on your official identity must appear on the licence, so make sure your documents match in spelling and order.
The photo
Taken in the last 12 months, in colour and sharp, against a plain neutral background, showing your full head from the top of your hair to the bottom of your neck, facing forward with your eyes visible. No sunglasses or tinted glasses, no head covering unless for religious reasons, and no selfies.
Incomplete applications
An application with missing information or an unpaid fee is not processed. Misleading information normally means refusal and can lead to prosecution.
